Description:
Assesses, plans, implements and evaluates the needs of patients for discharge planning activities.
Qualifications:
RN or Social Worker and knowledge of discharge planning and community resources. Two years of related experience in acute care or other environments and two to five years' experience as a discharge planner or case manager.
